My Father Has Gone Thr Chronic SDH (15mm Of Thickness

My father has gone thr Chronic SDH (15mm of thickness with 10.7 mm midline shift to left) surgery 2days ago...it was successful...right now he has got sense, but the issue is he has gone thr DVR operation 6 yrs back so he has to take 7.5 mg warfarin to maintain PT inr in control.
Now his current PT reading is 1.5 inr after brain surgery..so doctors had to give helparin to maintain pt inr,but that may cause bleeding in brain again....is there any way both bleeding in brain and heart pt inr can be controlled...???
