My Female Cat Is 16 Yrs Old, The Area Around

My female cat is 16 yrs old, the area around her anus is swollen and sore to the touch... It's not her actual anus where she poops from but the area around it ... She seems to be in pain when I lift her up by putting her tail under her. I have seen her try to poop twice today and nothing. Any suggestions. Thank you
