My Foreskin Seems To Have Swollen. It Also Seems Dry.

My foreskin seems to have swollen. It also seems dry. When I get an erection, my penis head seems to be too big as it extends past my foreskin. I noticed what seem to be little cuts on my foreskin & only until now they have bled. I also, have an itching sensation & at one point have had many white bumps on my penis head. I have used hydrocortisone cream & it seems to clear up. However, once I stop using it, after a few days, it comes back.
