My Grandson Is Three Years Old And For The Last

My grandson is three years old and for the last few months he is constantly sticking out his tongue, both up to his nose or down to his chin. I noticed that my granddaughter his 8 month sister is also starting to do it.
I have asked my daughter if she has mention it to their pediatrician but she gets upset.
What does this mean?
