My Husband Is Diabetic On Insulin But Still Has High

My husband is diabetic on insulin but still has high numbers. He's been experience more and more frequent bouts of dry heaving, which sometimes leads to vomiting if he has eaten within a few hours. He also has his hearing affected; he said it's as if he is underwater. What can this be?
