My Husband Is In The Final Stages Of Dementia, Bed

My husband is in the final stages of Dementia, bed bound, non communicative can do nothing for himself. He has a little fluid retention in his ankles in the last few days. He has had minor problems with a retracted foreskin which I can always manage to put right. This morning his foreskin is NOT retracted but is puffy looking but NOT inflamed at all. Is this to do with fluid retention? Thank you
