My Husband Is On Blood Thinners Due To Chronic Heart

My husband is on blood thinners due to chronic heart condition. However, the slightest bump or scratch causes a lot of bleeding. Now there are sores erupting on his arms that swell and bleed then heal over only to start the cycle again. Any suggestions?
