My Husband Is Quick To Anger, Narcissistic And Has Lately

My husband is quick to anger, narcissistic and has lately had issues comprehending me when we talk. He had cervical spine surgery (c5-c6) last October, and is having problems again, possibly related to c5-c6, but he also has severe foraminal stenosis at c6-c7 that wasn't repaired. Could his anger and confusion be related to this? Where can I get help? He says the issue is me, yet everyone around him has noticed his behavior issues.
