My Husband Start Complaining Of Itch And Some Swelling On

My husband start complaining of itch and some swelling on srotum sac ..I thought was rash i got a cream ..he put on the cream a couple times. we spent all day on the beach in hot sun. It is swelling more now. Not hurting and urine normal, but red and swelled. What should I do?
