My Husband Was Released From The Hospital Three Weeks Ago

My husband was released from the hospital Three weeks ago where he contracted C Diff. He has completed ALL the antibiotics but is having continue soft stools today. How do we know I'd he might have a relaps of this horrible disease? His reason for hospitalization was colon cancer.
