My Mom's Blood Pressure Is 190 Over 75. She Is

My mom's blood pressure is 190 over 75. She is 86 years old and its been that high before - usually when she anxious. She broke her arm a week ago Sunday, but insisted on going back to her own home two days ago. She is on blood pressure medication and just took a shot of her nitro spray - like a minute after she saw that reading - this is the first time its been that high in about 2 years though. Is this an emergency? Do I take her to the hospital?
