My Moms Cat Is 16 Today He Seemed Fine But

My moms cat is 16 today he seemed fine but this afternoon a few times he has had like a head tremor (like shaking his head no) very subtle and if u pet one side of his neck he immediately picks up his back leg like he's trying to kick/scratch at his neck not acting himself ??
