My Mother Is 97 Years Old And Lives With Me,

My mother is 97 years old and lives with me, this passed week her B/P had been very high up as 212/91 so her doctor gave her water pill hydrochlorothiazide that send her to the ER for fainting. The test result showed low sodium and low potassium, I noticed signs of dementia lately with my mother. Disoriented, confused, behavior problem like defiance, refusal to follow what is ask of her. Should I ask her primary doctor for name da for example? She just started lexapro tonight. I take care of her meds now to make sure she takes them and i fix her meal on a plate to see the amt she eats, is there anything more i can do for her. She don't call me when she want to potty and she has a couple of falls due to dizziness and light headed feeling according to her. She feels like that when her B/P is high.
