My Name Is Steve, I Am A Nurse , I

My name is Steve, i am a nurse , i have a patient that beginnings to sleeps all the times His Dx. DMII/HTN/CKD. I refereed him to a Psychologist/cardiologist/Nephrologist, and to me nobody seems to know what is going on, the last MD is suggesting Palliative care today, can you give me an idea or advise.
