My Niece Can't Make A Decision About Anything...what Gum To

My niece can't make a decision about anything...what gum to choose from the counter, what school to go to, what major, what to wear for her senior picture, what to eat for dinner... she also has anxiety attacks during tests when she knows the materials, and she's afraid to get her drivers license. Her father is either manic depressant or bipolar. Are these signs of a bigger problem? Please advise. thanks.
