My Son 33 Yrs Old Fell Hard On His Left

My son 33 yrs old fell hard on his left side onto a wooden stool he lost his breath has a knot the size of an orange it's been a week he tells me he stopped urinating blood he still has a hard time taking a deep breath very tender to the touch I've finally convinced him to seek medical attention my question is should he pay for the X-ray or go straight to emergency for a ultra sound or Mir insurance is not active yet what is the worst we could expect?
