My Son Crushed His Big Toe With A 45lb Weight,

My son crushed his big toe with a 45lb weight, first they said reconstruction, then maybe have to remove tip of toe and now they are just cleaning wound and nail, said nothing more they can do, should heal on it's own, with lots of antibiotics not to get bone infection. My question is what happens with all the shattered bones in the toe and will he forever have pain? Only 28yrs old.
