My Sons Right Ear Became Cauliflower-like Without Any Trauma About

My sons right ear became cauliflower-like without any trauma about a year ago. He drained it himself over a period of several weeks. Now his other ear is having the same symptoms again without any trauma. The fluid coming out is clear and a little bloody. What's causing this and what should we do?
