My Wife Has Piles. Every 3months There Is A Severe

My wife has piles. Every 3months there is a severe issue of bleeding piles. I apply her anno bliss with nozel on. But that hurts her. She says i have caused fissure in her again because of the wrong way of applying anno bliss. Its nozel she says has caused damage. Is it possible to get a fissure because of its nozel. If yest what other methods should i try to cream her with ano bliss?
