My Wife Is 60 Years Old And Extremely Overweight. She

My wife is 60 years old and extremely overweight. She visits her doctor regularly and has elevated blood pressure. Lately she has exhibited a big change in behavior. She is very irritable, forgetful, and she has isolated herself on the sofa with little contact with friends of family. Actually she has alienated most of her family and she has become very short and irritable with our 2 grown daughters. She has assumed some very extreme political viewpoints and cannot be around anyone that does not agree with her. Also she has many internal aches and pains and now some hand tremors. I am very worried and am not sure what to do. Family members and friends have asked me what is wrong with her and I don't know how to respond. She is irritable with me as well. At this age her father became very withdrawn and isolated and her maternal grandmother had some form of dementia late in life.
Any suggestions?
