One Of My Relatives, 25 Years, Male, Does Not Want

One of my relatives, 25 years, male, does not want to go out, he stopped studies also since last three years . Now a days he says he gets instructions from god in dreams like life is meaning less. Though we want to show him to a doctor, the issue is that he does not want to go to a doctor, he is at Shajapur, 120 Km away from Indore. How to make him to see a doctor and should we go to a psychiatrist or neurophysician ?
