Over The Counter Pregnancy Test Came Back Positive But My

Over the counter pregnancy test came back positive but my ultrasound showed only a dark sack with no development within. What could this mean? Obgyn said it could be too early (6 weeks pregnant) or a possible miscarriage. How does that happen if the egg was fertilized?
