Recently I've Been Suffering From What I Believe To Be

Recently I've been suffering from what I believe to be symptoms of either be GERD or a Hiatal hernia i.e. heartburn that worsens throughout the day/when I lie down, chest pains and some tenderness on the front-bottom part of my neck on the right side (just above/on my collarbone). These symptoms lasted around 5-6days and then seem to have pretty much stopped, however, I am now experiencing slight pain/discomfort on the back right of the roof of my mouth (in/around the soft bit) when i swallow. I'm just looking for some information and any input is appreciated.
