Renal Pelvis Extended To Proximal Ureter On Ultrasound. Back Pain.

Renal pelvis extended to proximal ureter on ultrasound. Back pain. Red blood cells and leukocytes in urine, but no bacteria on culture. Cystoscopy and retrograde pyleogram showed no blockage in ureter. CT showed no evidence of stone disease. 50+ female. What could be causing this?
