Right In Front Of My Ankle Bone On Each Foot

Right in front of my ankle bone on each foot on the outside part of the foot is a swollen area. It's been like that a few years now. It never goes down. I also have pain / ache in both of my largest toes. This pain is off and on and hurts with movement only. I get pain sometimes on top of the foot also. I've tried to pin point what may be causing the different areas of pain or the permanent swelling or mass by my ankle and can't.
