Since 2006 I Have Been Having Double Vision With Numbness

Since 2006 I have been having double vision with numbness and tingling in my face, with some numbness in my right arm and fingers. It happened a handful of times the first year and has progressively got worse. Now I have balance issues, dizziness, memory loss, lower back pain and muscle spams sometimes. How can I get checked for MS? What test can be done?
