Since I Have Had Abdominal Surgery On June 8, Due

Since I have had abdominal surgery on June 8, due to adhesions and 2 small mass my Inr goes as high as 8.5-9.5. I just had blood work last Friday levels were 8.5. I have Systemic lupus and my veins are very bad which at times it takes anywhere from 4-8 draws. I have been on coumadin since 2000 when I had a mild stroke which affected the right side for about a month, luckily through therapy while in the hospital I regain the right side. I don't know what to do and I know the foods and meds which can affect my levels. Any new foods, meds etc., which have recently come up?
