Sir, I Have Been Severely Constipated Since My Childhood. Now

Sir, I have been severely constipated since my childhood. Now I am 30 years old and it has gone worse over the years. I drink adequate water and eat adequate fiber and fruits also. I dont use laxatives because I dont want to be dependent on them. But lately I took stool softener docusate but still i feel soft stool stuck in my rectum. I cannot evacuate for which I use tissue to manually evacuate and clean my rectum. I feel very sick and uncomfortable. Please advice me.
