Sir, One Of The Incissor Of My Upper Jaw Is

Sir, one of the incissor of my upper jaw is half broken , but It doesn't cause pain or other problems. On the lower jaw, the first molar on left side has developed a deep large cavity. I also have about 3mm gap between the two incissors on upper jaw. Other things are fine. How much do you think is my dental score (points). If its less than 14 then what shld I do to increase it. I wish to join army so it would be very kind of u if u help
