So Marijuana Just Became Legal In My State. For Medical

So marijuana just became legal in my state. For medical use only that is. Under certain criteria and illness.
I've had a heart murmur since birth hasn't been checked since about age 7, so not much of a worry.
Also have mild-severe back strains from exerting too much energy sometimes even spasms or twitches.
And the big one is the build up of stress and a short temper.
Marijuana has been my helpful friend since 9th grade keeping me calm, focused and motivated.

So my question is with my underlying 'problems' and pains is there a certain criteria I should ask my doctor to label or prescribe me as?
