Suggest Medication For Severe Fatigue Due To Low BP

My husband and I went out for along bike ride today. After he came home and felt severely fatigued. We checked his blood pressure and it is extremely low...87/48 sometimes it goes up just a little then back down again. He is on high blood pressure medicine and this has never happened bfore. What is wrong?
