Suggest Medication That Helps Longer Time With Break Through Pain

i have been on the fentanyal 200 mcg / hr patches for three years...along with lortab 10/325 (10/500 at first then it was reduced) everything was fine. My doctor didn t like all that pain meds so he reduced the patches to 150mcg/hr and increased the lortab that was not not helping much any more to percocet 10/325. The percocet causes its own problems and I asked him to increase the diazepam because it helps more than any pain killer since hes reduced the patches...is there a happy med that will help longer when the break thru pain becomes very bad?
