Suggest Remedies For Fissure On My Foreskin

Hi, I have fissures on the top part of my foreskin around the head in like a diamond shape going away from my penis in direction. The skin around these is flakey and comes off quite easily to reveal pink skin. The fissure cuts crack and open and very little bleeding sometimes. I have sexual contact with a long term partner, and this problem is starting to get a little worrying, Any help?
