Suggest Remedy For Difficulty In Retracting Foreskin

My semen stays inside the foreskin and shaft all round, and restricts foreskin retraction, though with great difficult I pulled back the foreskin with help of coconut oil and cleaned the shaft; again it's forming/recurring and it's too difficult to pull back my foreskin with severe burning sensation
How to prevent this?
Should I visit a doctor for this
