Suggest Remedy For Radiating Pain From Left Upper Back To Rib Cage

I have copd (late stage). Suddenly having severe pain in left upper back that radiates to under the rib cage (under left breast) . Have been off of steroids and antibiotics for 4 weeks. Seems to affect left shoulder (which has had a rotor cuff repair) and up the left side of neck. I do use a tens machine and pain patches which gives me short term relief but tylenol and advil are not helping. Should I use a heating pad or a massager?
