Suggest Remedy For Wound Near The Buttocks

Hi doc. I have a wound like thing above to my butts and center to them.it is there since 5 months.recently it induced a lot of pain and it was opened when i slept and through it meat like thing came out with blood. What is it doctor? How can i take care of it? I'm so sacred. I'm male age 21l
