Suggest Treatment For UTI While Having Renal Hematuria Syndrome

I have a urinary tract infection that has not responded to Bactrim. Finished the course 4 days ago and pain is now so bad. I am 64 yo, have Renal Hematuria Syndrome along with other chronic conditions. Have tried Ural sachets, cranberry juice, heat packs, increased water intake, paracetamol.
