Suggest Treatment For An Irritated And Torn Meatus

About 2 months ago, my meatus accidentally was torn about 3/16", about half of that going towards the top of the opening and about half going towards the bottom. It gets irritated quite easily, I have had to wear gause ever since because the friction of my underwear again my skin as I walk is enough to pull one side away from the other which hurts. I can't urinate without holding back because it aches/stings afterwards.

I have seen a doctor who gave me some antibiotic cream to put on it. The injury never bled, and the doctor thought it looked normal (I guess because there's a lot of variance in this region for people).

My worry is that it'll stay this way. I'm fairly sure I won't be able to have sex again if that happens - I can't even wear my clothes w/o gause. I can't exercise or do anything vigorous because it jostles everything around which is irritating as well...

It's been 2 months. What options are there? I'm concerned this like an accidental meatomy and I really want to know what my next steps would be say 2 months out if I'm not seeing improvement.
