Suggest Treatment For Back Pain And Paraesthesia

Hi, I am going to be going to the spinal.center in Duke, NC this week. The receptionist told me to send him an email regarding my diagnosis and other information. I do not know how to present myself or all of the spinal diagnosis that I have because I have been denied and been so degraded in the the past. How should I present the letter so that it sounds serious and that I am scared. I do not want to return home until my back and the Parasthesia are treated. I am honestly to tired to even fight for my health anymore. I need him to fix me but I do not want to be seen for 15 mins and then sent home. So my question is how to make him look at the Dx I have and to try to make a definitive DX?
