Suggest Treatment For Dent Above The Anal Opening

my boyfriend just showed me a whole about an inch above his anus,and he says it bleeds all the time. I looked at it and it almost looks like a deep tear, but yet like the walls are pushing through it. I looked up pics and it looks like it can be a pilondial sinus. Is this something he needs to go see a Dr for, or are there ways to heal it on your own? He says it's been ther for years. I'm scared, but don't want to freak him out. Should I be worried? Could it be more?
