Suggest Treatment For Ear Pain While Having Diabetes

I had sharp ear pain in my left ear, and a dull ache in my right. I have seen my PCP in the past with thye same issue, she said there was fluid and tubes would help. I saw a ETN yesterday, she said I have TMJ, but from what I read it does not seem to fit. i also have type 1 diabetes, i would like a second opinion but I am not sure what type of doctor I should see next. The pain is intense.
