Suggest Treatment For Fatigue, Ascites, Edema Fingers And Legs

Our friend is quiet ill and was previously healthy with no chronic conditions. It began with fatigue, advanced to ascites which has required several treatments of draining the abdominal fluid. He has generalized edema mainly in the fingers with a lesser amount in the legs and feet. Multiple ct's, mri's, pet scans, blood work ups have shown nothing. he does have at times an enlarged spleen which returns to normal. He is not a drinker, or smoker. Now he is having neuro changes and today he is anemic and they suspect an abdominal bleed. no cancer markers are present. Could this be a parasite? What could it be?
