Suggest Treatment For Flaking And Itching Of Scalp With Hair Loss

hi sir i am having this hair problem since long, flakes and itching followed by loss of hair, tried keeping hygene but this situation never stops it comes back, went to dermatologist, given topgain for use but its been 4 months no much improvement, please advise whom to consult for a permanent solution to my problem, hiar line is receeding very fast.
