Suggest Treatment For Hard,red And Itchy Bump On Leg

I've got a small hard, reddened, raised 'bump' on my leg. It appeared years ago (about two years ago) as a small dry irritating patch, but it didn't do anything and I just put hydrocortisone cream or body cream on it. Just recently it has changed its form and how is quite hard, quite red and itches like fury all the time. Any suggestions?
