Suggest Treatment For Hearing Problem

I have serious hearimg problem in both ears:
presence of const noise,
pain in inside & surroundinding area of ears,specialy while listening,
previous investigation depicted dried vein: & internal collision generating noise.
Real causes are not yet establised.
Antibiotics & other medicion have failed to settle theproblem.
Under the circumstances,what examination/treatment should be followed?
