Suggest Treatment For Herniated Disc And Pinched Nerve

I have a twisted turn pelvis that is pressed into my spine and a herniated disc pinching my sciatic nerve. I've been in excruciating pain from this since January 4 of 2015 I've noticed that a dent has appeared in my front left thigh. Atrophy? the pain haw worsened and I'm scheduled for pain management on 17 March. today I cannot walk. I'm concerned that my condition is worsening. A lot of pain seems to be isolated in my hip but the confusion for me now is what is this dent? I'm attaching a picture.
