Suggest Treatment For Itching On Anus And Scrotum

I have an itching both on my anus and scrotum for more than five years. I could not get rid of it! It itches me very very very much especially in when the day gets hot or I sit long. I itch it at times till it bleeds. I have tried some anti-biotic creams , What do you recommend me?
