Suggest Treatment For Memory Loss

I have a 61 year-old cousin with multiple serious illnesses. She has been hospitalized (this time) since early December, 2014 following a third incident of toxic medicating, which has effected her ability to remember. I d like to create a chart of interactions for her currently active, 21 medications.
