Suggest Treatment For Pain And Pressure In The Shoulder And Collar Bone

I had rotator cuff surgury 3 weeks ago and I fell forward and had to catch myself and jamed my arms into the steps. I called my doctor and they said it was like starting over after the surgury, and to take Advil that was on Monday. It is now Wednesday and I am having alot of pressure in the shoulder and much pain, but also the pain has moved into my collar bone and hurts when I breath. Is that normal?
