Suggest Treatment For Painful Cystic Acne Around The Mouth

I suffer from cystic acne aroun my mouth, it is very painful, and embarrassing. I've tried retinal creams, and a lot of over the counter creams, gels, washes and whatnot. They are large and I'm over it. My whole chin is scared up from it. What can I do? I almost think it's hormonal.
